Clinical Pharmacist Specialist – Oncology – Ambulatory/Outpatient (POEM Program) 40hrs/wk

Shift: Days

General Summary:

The clinical pharmacist specialist (CPS) is responsible for the day-to-day operations of the Pharmacy Clinical Services, under the direction of the Pharmacy Clinical Coordinator and the Director of Pharmacy Services. CPSs provide consultation to physicians, nursing staff, and patients regarding the safe and effective use of medications in all patient populations to promote positive outcomes. They participate in the core distribution, disposition, security, preparation, and procurement functions of the Pharmacy, in addition to providing education to pharmacy, medical, nursing and general staff. Additionally, CPSs train, supervise, and direct the clinical efforts of Clinical Pharmacists, students and support personnel. CPSs participate in quality initiatives involving optimal medication delivery and use.

Job Requirements:

Required

  • Bachelor of Science in Pharmacy degree from an ACPE-accredited college of pharmacy (minimum) – and/or – Doctor of Pharmacy degree from an ACPE-accredited college of Pharmacy (preferred)
  • Pharmacist licensure in the state of Michigan (registered pharmacist & controlled substance license)

Preferred

  • BPS certification (e.g BCPS, etc.)
  • Significant institutional Pharmacy experience
  • Significant clinical Pharmacy experience
  • Completion of an institutional residency
  • Other certifications (e.g. BCLS, ACLS, etc.)
  • Extensive knowledge of HFAP standards

  • Demonstrates sound professional judgment.
  • Knowledgeable of software applications related to the job including:
    • word processing software (e.g. Word) - preferred
    • spreadsheet software (e.g. Excel) - preferred
    • presentation software (e.g. PowerPoint) - preferred

Essential Functions and Responsibilities:

  1. Ensures safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, protocols, directives, or pharmacy operation standards.
  2. Provides education and information on pharmaceutical care and drug therapy.
  3. Contributes to the quality and effective operation of the department.
  4. Actively Promote/Participate Safety and Quality Improvement
  5. Lead, conduct and/or participate actively in committees/meetings.
  6. Communicate effectively with both internal and external personnel.
  7. Performs other duties as assigned. These may include but are not limited to: Maintaining a current knowledge base of department processes, protocols and procedures, pursuing self-directed learning and continuing education opportunities, and participating on committees, task forces, and work groups as determined by management.
